Our Services

Access to services and resources

Choices for Care

A Care Coordinator provides an assessment to all potential nursing home applicants 60 years and over to discuss options for long-term care.

Adult Protective Services

The Illinois Department on Aging’s Adult Protective Services Program investigates & intervenes in allegations of abuse,

Community Care Program

Our Care Coordinators, certified by the Illinois Department on Aging, conduct comprehensive in-home assessments for seniors at least twice a year.
